Electronic Product Fulfillment Web Access Request Form

Intent to Remove MID Form

This form can be used by eVS shippers to request delinking of Child MIDs assigned to their Master MID that have been issued to their customers they are no longer affiliated with.  Master MID owners making this request should provide the Child MID user’s name, address and contract information.  Upon review by USPS and after the Child MID has not had any activity on it within 120 days, the Child MID will be delinked.  The completed form can be submitted to eVS_HQ@usps.gov with “Request to Remove Child MID (MID #)” in the subject line.
